The Birch, Chatterton, and Norman J. Levy Lakeside Elementary Schools of the Merrick School District will hold registration for the September 2019 kindergarten class from Monday, February 4, 2019 through Friday, February 8, 2019.

Children who will reach the age of five on or before December 1, 2019, are eligible. To register a child, parents are requested to call their neighborhood school between the hours of 10:00 a.m. and 2:00 p.m. to make an appointment. If you are unsure of your child’s neighborhood school, please call the Transportation Office at 516-992-7285.

On the day of registration, please bring your original birth or baptismal certificate (with a raised seal of either document). Please also bring with you health forms signed by your physician, indicating the completion of all required immunizations. Please refer to “Important New Information on Immunizations” section below to learn about required immunizations.

In order to substantiate your residency, when you meet with us, you must produce a deed or a lease, plus a driver’s license, and three utility bills (e.g. telephone, PSEG Long Island, National Grid, heating oil and cable/internet bills).

You will receive a packet of paperwork in the mail prior to registration week. When you come to the school for your meeting, please bring your completed packet with you. It is not necessary to bring your child to your registration appointment.

IMPORTANT NEW INFORMATION ON IMMUNIZATIONS

This is an update of the requirements regarding immunizations for school entry. In order to ensure that children entering and attending school are adequately immunized against vaccine-preventable diseases, the New York State Department of Health Immunization Program, in accordance with ACIP guidelines, revised their immunization requirements for school entry:

  • 4-5 doses of Diphtheria, Tetanus, Acellular Pertussis*
  • 3-4 doses of Polio**
  • 2 doses of MMR (Measles, Mumps, Rubella)
  • 3 doses of Hepatitis B
  • 2 doses of Varicella vaccine or documentation of proof of disease

All immunization documents are required at kindergarten registration. Our school nurses will review all documents and will inform you if your child requires any additional vaccines in order to begin school in September. In the event these requirements are not met by the first day of school, according to NYS law, you are permitted a 14-day grace period as stipulated under PHL Section 2165 (measles, mumps and rebella requirements). If these requirements are still not met after the grace period, your child will be excluded from school until the requirements are met.

*  If the 4th dose of DTap was administered at age 4 or older, the 5th (booster) dose of DTaP vaccine is not
